SBLC notes, a 1903 identification, and extra time on LEAP are useful history. They are not the application. College Board and ACT want their own file: a current comprehensive battery, a disability history, and a specific ask — extra time, breaks, a reader, a separate room.
Work backward from the national ACT date you actually need for TOPS, not from “sometime junior year.” A report finished the week of the deadline is how people sit standard time and try again in the spring.

Before you book, ask whether the diagnostician has written ACT or SAT packets and what they put in the recommendations. You need an identification that matches the request, scores that show why standard timing fails, and wording you can copy onto the form. A score table with no ask is a wasted invoice.
