Ask what they want: cognitive only, cognitive plus achievement, a named instrument, how recent. Then forward that email to the diagnostician. A school-facing ARD report is the wrong product for most independent-school files. The reverse is also true.

What the school can actually read
A short parent-readable narrative. Scores they named. A clear statement if a composite is misleading. If the child needs classroom accommodations after they enroll, say that at intake so recommendations are written for a private campus, not for an HISD ARD.
